I have misplaced the codes for installing SolidWorks 2009.  Where can I get a copy of the unique code for my school?

This unique code will only made available to the school management/DCG teacher. Students can only receive the school license ID for the Student Design Kit (SDK) from their DCG teacher.
Is it possible to get the service pack 5 upgrade on a disk or is the internet the only method for downloading the upgrade?

You will still have to download from the internet as T4 does not keep a copy of service packs on disk. When you are downloading a service pack however you should be given the option to download to disk which would give you the opportunity to upgrade a range of computers from this disk.
